If this is the first time you have signed in, or if you have previously signed in but have not yet created a case, or if you have deleted all your cases, you are prompted to create a case:

To create a new case:
The Case Details form is displayed:
You must complete all the fields on both pages of the form.
Enter up to 50 characters of free format text to describe the case. Give the case a name that will enable you to easily identify it.
Make a selection from the drop down list box depending on who the patient is. This is useful when, for example, you might set up one case for yourself and another for your spouse.
Select either male or female.
Enter the age of the patient, in the range 0-130.
Select either Yes or No, depending on whether you/the patient are happy with the treatment received so far from the health service?
Make a selection depending on how many times the case has been discussed.
If you/the patient have a confirmed diagnosis, select it by using the drop down list boxes. Three levels of selection are available, enabling you to make a selection based on your current knowledge of the disease/illness.
For example, if you know that you have a problem with your digestive system, but no more than that, select K00-K93 - Diseases of the digestive system from the first (top) drop down list box, and do not select anything from the other two drop down list boxes.
If, in addition to knowing that you have a problem with your digestive system, you know that the problem relates specifically to your liver, select K70-K77 - Diseases of the liver from the second (middle) drop down list box, and do not select anything from the third drop down list box.
If you know the specific problem you have with your liver, make that selection from the third drop down list box. In this situation, you will have made a selection from all three drop down list boxes.

Make a selection depending on whether the patient's condition is getting better, getting worse, or staying the same.
