Case name

Who is the patient?

Make a selection from the drop down list box depending on who the patient is. This is useful when, for example, you might set up one case for yourself and another for your spouse.

What is the gender of the patient?

Select either male or female.

How old is the patient?

Enter the age of the patient, in the range 0-130.

Are you/the patient happy with the treatment received?

Select either Yes or No, depending on whether you/the patient are happy with the treatment received so far from the health service?

How many times has the case been discussed with a healthcare professional during the past six months?

Make a selection depending on how many times the case has been discussed.

Do you/the patient have a confirmed diagnosis for the disease/illness?

If you/the patient have a confirmed diagnosis, select it by using the drop down list boxes. Three levels of selection are available, enabling you to make a selection based on your current knowledge of the disease/illness.

For example, if you know that you have a problem with your digestive system, but no more than that, select K00-K93 - Diseases of the digestive system from the first (top) drop down list box, and do not select anything from the other two drop down list boxes.

If, in addition to knowing that you have a problem with your digestive system, you know that the problem relates specifically to your liver, select K70-K77 - Diseases of the liver from the second (middle) drop down list box, and do not select anything from the third drop down list box.

If you know the specific problem you have with your liver, make that selection from the third drop down list box. In this situation, you will have made a selection from all three drop down list boxes.

Tip: If you cannot find your disease/illness by navigating through the drop down list boxes, try using the Diagnosis Locator. The link for this is directly beneath the third (bottom) drop down list box.

Has there been any change in the patient's condition?

Make a selection depending on whether the patient's condition is getting better, getting worse, or staying the same.
